Complaints Against an Individual Process Server
A person desiring to make a complaint against an individual process server shall use the official complaint form approved by the Board and provided below.

Other than complaints by members of the judiciary, the complaint shall be signed under oath. Each complaint shall include a succinct statement of the nature of the complaint along with all pertinent documentary evidence attached thereto, and submitted to the Board’s mailing address provided below. Be advised if a complaint is submitted via fax, original documentation is still required before an investigation will be undertaken.
