Application Fee: $225.00
Late Fee: $112.50 if received less than 45 days before expiration. Late fee is incurred if complete application is received 44 days or less before expiration. To reinstate certification if a lapse occurs, you must pay all prior fees incurred (Application Fee $225.00 + Late Fee $112.50 + Prorated Fee (if applicable)).
