Required Reporting for Calendar Year 2012

Guardianship Certification Board


Texas Government Code Chapter 111, Texas Probate Code Sections 697 and 697A, and Rule X of the Rules Governing Guardianship Certification impose requirements for county clerks, the Department of Aging and Disability Services, guardianship programs and private professional guardians to report certain information to the Guardianship Certification Board. In addition, Probate Code Sections 697 and 697A require guardianship programs and private professional guardians to provide certain information to county clerks. Reporting requirements are the same as for calendar year 2011.

All required reports are due to the Board and the county clerks, respectively, no later than 01/31/2013 for the calendar year ending 12/31/2012. Reminder letters will be sent the first week in January 2013. Click the appropriate link below for the text of each reminder letter.

Please click the appropriate links below for a summary of reporting requirements and for the appropriate form. Do not use forms from a previous year - use only the new forms dated 12/31/2012. Certified or provisionally certified guardians who are not employed by a guardianship program or by DADS, and who do not meet the definition of private professional guardian, should use the Other Certified Guardians reporting form.

NOTE: private professional guardians must submit original, signed and notarized reporting forms to the Board by mail or in person. All others may be submit reporting forms by mail, in person or by e-mail as shown on the forms. E-mail submissions are preferred.
